COVID-19 Waiver

The one time waiver must be signed by all attendees considering to visit the facility for prayers. 

 

The waiver can be submitted using below options

1.  Print, complete, and email to tricityislamiccenter@gmail.com. In the subject line, type “One Time Waiver.”

2.  Print, complete, and bring it with you when you come for Salat. 

3.  Fill & submit on site.
